[기자회견] 생명정보, 안전 팔아 의료민영화 추진하는 '규제샌드박스' 규탄
지난해 12월 유엔 기후변화협약에서 체결된 ‘파리협정’을 두고 전 세계 언론은 ‘화석연료 시대의 종언’이라는 헤드라인으로 소식을 전했다. 심각한 기후변화를 막기 위해선 산업혁명을 이끌었던 화석연료 의존에서 긴급히 벗어나야 한다는 것을 윤리적이고 법적인 새로운 규범으로서 채택했기 때문이다. 국제 조약이라는 차원을 넘어 파리협정이 우리 삶에 어떤 의미를 갖는지는 이제부터 곱씹어야 할 문제지만, 값싼 화석연료에 취해있던 시대와 결별해야 하는 ‘신 기후체제’가 본격화됐다는 강력한 신호로 작용했다는 것은 분명해 보인다.
신호는 중요하다. 특히 에너지 문제와 같이 예측 불확실성이 높고 경제적으로 민감한 영역의 경우 더 그렇다. 게다가 ‘석유 한 방울 안 나오는’ 한국처럼 화석연료 대부분을 해외 수입에 의존한다면 더 말할 것도 없다. 안보라는 측면에서도, 우선 떠올려야 하는 항목은 이제 국방비 대신 에너지와 식량이어야 할지도 모른다.
신호탄이 울렸는데, 우리는 뛸 준비가 되었을까. 같은 신호를 들었지만, 반응의 온도차는 존재한다. 가령, 얼마 전 다보스 세계경제포럼(WEF)에서 세계 주요 기업의 경영자들을 대상으로 사업의 위협요인을 조사했는데, 기후변화와 환경위기는 낮은 순위로 나타났다. 그 대신, 경영인들이 꼽은 최대의 우려 요인은 ‘과잉규제’였고, 정치적 불확실성, 사이버 공격 등도 그 뒤를 따랐다.
규제완화는 한국뿐 아니라 세계 업계들이 공통적으로 보내고 싶은 1순위 신호라는 것이다. 얼마 전 한 정부 관계자로부터 ‘기후변화’라는 표현이 곧 ‘규제’를 연상시키기 때문에 정부 내에서는 이 단어를 기피하는 분위기라는 말을 들었다. 기후변화 대응이 불가피한 흐름이 된 가운데 기업에 부담이 가중진다면, 반대급부로서 새로운 규제완화가 필요하다는 논리다.
“에너지규제 다 푼다.” 최근 정부가 발표한 ‘에너지 신산업’에 대한 언론이 보도한 제목이다. 한전이 독점하던 전력 거래를 개인과 민간 사업자에게도 개방해 새로운 에너지 시장을 만들겠다는 것이다. 정부 설명대로 소규모 태양광을 설치한 개인이 남는 전기를 이웃에게 판매할 수 있는 등 재생에너지 확대를 위한 긍정적인 내용을 포함하고 있다. 화석연료와 핵발전 중심의 기존 전력 시스템이 재생에너지와 에너지 효율기술에 본격적으로 문턱을 낮추겠다는 신호는 좋다.
하지만 재생에너지에 유리한 시장을 만들겠다는 신호만으로는 큰 파장을 기대할 수 없다. 현재 재생에너지의 정책 목표나 지원제도가 너무나 의욕적이지 않기 때문이다. 게다가 재생에너지가 획기적으로 성장하기 위해선 석탄을 비롯한 화력발전과 핵발전 비중의 축소, 전기요금 정상화 등을 통한 전력 수요관리가 전제돼야 하지만, 이런 핵심적인 부분에 대해서 정부가 전향적인 변화를 선택하기를 주저하고 있다.
우리에게 정말 필요한 것은 에너지 효율화와 재생에너지 활성화라는 명확한 신호다. ‘에너지 신산업’이란 정도의 정책 신호로는 역부족이다. 기후변화의 시계는 이 수준의 대응 속도로 행동하는 것을 기다려주지 않을 것이기 때문이다. 세계 재생에너지 전환을 이끌었던 헤르만 셰어의 말대로 “모자란 것은 재생에너지가 아니다. 빠듯한 것은 시간이다.”

사회복지연대_
공공의료 강화를 위한 새로운 모델, 민간병원의 공공병원화
부산은 대표적으로 건강이 나쁜 도시이다. 서울과 비교했을 때 기대수명이 많게는 9.7세까지 차이가 났으며(2014년 기준, 서울 서초구와 부산 동구의 격차), 피할 수 있는 죽음도 16개 구군 중 강서구를 제외한 15개 구군이 서울의 모든 자치구보다 높게 나타났다(2013년 기준).
하지만 아이러니하게도 의료공급은 과잉된 대표적인 도시가 부산이기도하다. 병상수가 전국에서 1~2위를 다툴 정도로 많으며 요양병원 병상의 수는 전국에서 단연 1위를 차지하고 있다. 병원은 많은데 왜 건강이 나쁠까? 그 해답을 공공의료에서 찾기로 했다.

지난 9월 8일 건강정책학회와 부산대학교 공공보건의료정책센터 공동주최로 “의료의 공공성 확대·강화 어떻게 할 것인가? - 부산 침례병원 사태를 중심으로” 토론회가 부산일보사 소강당에서 개최되었다. 건강불평등 해소 운동을 진행하고 있는 사회복지연대는 지난 7월에 파산한 침례병원 사태를 공공의료강화의 기회로 삼기위해 ‘민간병원의 공공병원화’라는 공공병원 확대의 새로운 모델을 고민해 왔고 침례병원 파산에 따른 새로운 공공병원 설립을 위한 부산시민대책위와 함께 토론회에 참여하였다.
토론회에 참석한 부산의대 윤태호 교수의 발제에 따르면 부산은 7대 특·광역시 중 공공의료 수준이 취약하며(2015년 기준: 의료기관 수 6등, 병상 수 5등) 시민의 건강수준을 개선하기 위한 정책을 수행하는 것이 어려운 실정에 놓여있다.

때문에 공공병원의 확대가 필요한데, 의료공급과잉인 상태에서 새로운 병원을 짓는 것 방법보다는 민간병원을 공공이 인수(투자)하여 공공병원을 확충하는 방안이 효율적이며 600병상규모의 대형병원인 침례병원이 그 첫 사례로 적합하다는 발제가 이어졌다.
이후 토론에서는 성남의료원의 사례를 들어 주민들이 간절하게 원하고 자랑할 수 있는 공공병원, 민간병원이라 할지라도 민주적으로 운영될 수 있도록 주민이 이사회에 참여하는 병원 등 공공성 확대를 위한 병원의 미래상에 대한 토론과 침례병원을 공공이 인수하기 위한 현실적인 방안과 한계 등의 토론들이 이어졌다.
토론회의 좌장을 맡았던 김용익 전 의원은 민간병원의 공공병원화는 향후 공공의료를 강화하는데 필수적인 방법이며, 민간병원을 공공병원화 하는 침례병원의 사례와 주민들이 참여하고 자랑스러워할 수 있도록 올바른 공공병원 운영을 추진하는 진주의료원의 재개원의 사례를 함께 고민하며 부울경 지역의 공공의료를 강화시키기 위해 노력해야 한다고 이야기했다.
사회복지연대는 지난 2013년, 부산일보사와 공동기획한 ‘건강최악도시부산’시리즈 이후로 부산의 건강불평등 해소와 공공의료 강화를 위해 노력해왔다. 토론회를 통해 이번 침례병원사태가 오히려 기회가 될 수 도 있다는 생각이 들기도 한다. 물론 민간병원을 공공이 인수하는 것이 쉬운 길은 아니겠지만 건강최악도시부산이 건강최고도시 부산으로 거듭날 수 있도록 사회복지연대는 계속해서 활동할 것이다.
복지세상을 열어가는 시민모임_
'2017 권리로 요구하는 천안시 사회복지 정책제안 토론회'
시민 정책참여과정을 통해 모은 시미느이 목소리를 천안시 정책으로 제안
참여예산복지네트워크(사무국 : 복지세상을 열어가는 시민모임)는 9월 19일(화) 권리에 기반한 시민 정책참여과정으로 시민이 직접 천안시에 필요한 정책 7건을 발표하는 ‘2017 권리로 요구하는 천안시 사회복지 정책제안 토론회’를 천안축구센터에서 진행하였다.
정책제안 토론회는 아동, 청소년, 노인, 장애인, 이주민, 자활참여주민, 대학생 등 7개 영역에서 158명의 복지 당사자와 사회복지기관․단체 현장 실무자가 직접 권리워크숍과 원탁회의, 정책모니터링에 참여하며 문제점과 개선방안을 도출한 결과, 최종 7건의 정책을 천안시에 요구하는 자리이다.
2017년 시민 정책참여과정은 아동부터 노인, 장애인 등 총 158명이 4월 인권교육을 시작으로 각각 8번의 권리워크숍을 통해 320여가지 결핍점을 생생하게 이야기한 후, 75명이 한 자리에 모여 ‘권리원탁회의’를 통해 16가지 주제 중 ‘내 삶에서 가장 결핍된 점’에 대해 세부토의를 진행하고 우선순위 투표를 거쳤다. 이후 네트워크 기관․단체와 당사자가 함께 정책모니터링을 통해 실태와 문제점, 개선방안을 도출한 후 최종적으로 ‘저소득층 중․장년층 1인가구 고독사 예방 및 보호대책 마련’ 등 총 7건의 정책제안서를 작성하였다. 제안된 제안서는 9월 초 천안시 각 해당 부처에 정책을 전달하고 부서별로 의견서를 수렴하였다.
정책별 천안시 해당부서 사전 검토의견 수렴은 올해로 4년째 시행하고 있는 과정으로 사전 검토의견을 요청하기 이전보다 시민들의 삶을 더욱 살피며 책임 있게 정책을 수행하는 등 긍정적인 효과를 나타내고 있다. 그러나 부서검토 결과 7건 중 4건이 미반영과 보류 결과를 보이며, 절반이 넘게 시민의 의견이 반영되지 못하였다.
특히 저소득층 특수촬영 의료비 지원의 경우 참여예산복지네트워크에서 3년동안 제안하여 2015년 천안시장 공약사업에 포함되어 조례제정 후 시행하고 있으나, 지원되는 질환의 폭이 좁아 2016년 말 기준 18.%의 예산집행률을 보이며 실질적으로 지원이 필요한 저소득층에게 제대로 이루어지지 않고 있다. 또한 저소득층 중․장년층 1인가구 고독사의 경우 기록적인 고령화와 가파른 1인가구 증가세와 맞물리면서 전국적으로 급증하며 사회문제로 떠올랐다. 현재 고독사 관련 공식적인 정부차원의 통계는 없는 상태이며, 이와 관련있는 무연고 사망자 통계의 경우 전국 222개 지자체 중 천안시가 6위로 지난해 27명이 무연고 사망자가 발생하였다. 이에 지자체별로 중․장년층 1인가구에 대한 전수조사를 실시하고 대책을 마련하는 반면 천안시는 아직 고독사 예방을 위한 조사나 대책을 마련하지 않고 있다.
참여예산복지네트워크는 토론회 이후 조례개정 등을 통해 제안한 정책이 실행될 수 있도록 간담회 등을 진행할 예정이며, 사회복지예산의 양적인 확대 뿐 아니라 실질적으로 시민들이 필요로 하는 정책이 될 수 있도록 활동하고자 한다.
국제 평화단체, 한반도⋅동북아 위기에 대한 외교적 해결 촉구

전 세계의 110개 평화단체들과 146인의 평화운동가들은 당면한 동북아와 한반도 위기에 대해 무력 사용을 반대하고 외교적 해결을 촉구하였습니다. 전 세계 평화와 군축을 위해 활동하는 이들은 이번 서한을 통해 지금의 위기가 오판과 우연에 의해 핵전쟁으로까지 비화된다면 전 세계는 핵참화를 피하기 어려울 것이라고 우려하며 △선제적 무력 사용 반대, △도발적인 군사연습 중단, △동북아비핵지대안 검토, △한국전쟁 종전, △6자회담 재개, △유럽연합의 외교적 협상 지원 환영, △외교적 해결 우선 등 7개 요구사항을 제시하였습니다.
이들은 이러한 입장을 담은 서한을 지난 9월 7일 남⋅북⋅미⋅중⋅러⋅일 등 6개국 대통령(또는 수상)과 유엔주재 대표, 유엔안보리 회원국, 안토니오 구테레스 유엔 사무총장 그리고 페데리카 모게리니 유럽연합 외교안보 고위대표에게 보냈다고 밝혔습니다.
이번 공동서한에는 퍼그워시 회의(Pugwash Conference), Campaign for Nuclear Disarmament(영국), Coalition for Peace Action(미국), 원수폭금지협의회(일본), 참여연대(한국) 등 전세계 24개국에서 활동하는 평화단체들과 1976년 노벨평화상 수상자인 메어리드 코리건 매과이어(Mairead Corrigan Maguire)를 비롯한 대표적 평화운동가들이 함께 하였습니다.

▣ 공동서한 국문
동북아 상황의 외교적 해결을 호소합니다
전 세계에서 평화와 군축을 위해 활동하는 ‘어볼리션(Abolition) 2000’의 회원단체들을 대신하여 미국과 북한에 동북아가 직면하고 있는 전쟁 직전의 상황에서 한 걸음 물러나, 대신 전쟁을 막기 위한 외교적 접근법을 택하라고 촉구합니다.
우리는 군사 갈등이 발발하는 것을 막고 나아가 근본적인 갈등을 해결하기 위해 즉시 협상에 나설 것을 요구합니다. 그러한 협상은 양자 간은 물론 중국, 일본, 북한, 러시아, 한국, 미국을 포함하는 새로운 6자회담 틀을 통해 이뤄져야 합니다.
북한의 핵‧미사일 능력을 두고 고조되는 긴장과 군사 갈등 위협은 외교적 해법이야말로 필수적이며 최우선임을 다시금 일깨워줍니다. 전쟁 위기가 높아지고 오판과 우연, 또는 의도에 의해 핵무기가 사용될 수도 있다는 사실은 매우 끔찍한 일입니다.
1950년에서 1953년까지 이어진 한국전쟁에서 목숨을 잃은 사람들의 수는 남북, 중국, 미국 등 다 합해 총 300만 명을 넘습니다. 만일 전쟁이 다시 발발한다면, 특히 핵무기가 사용된다면 사망자 수는 훨씬 더 많을 것입니다. 한반도에서 발발하는 핵전쟁은 핵 참사로 전 세계를 집어 삼킬 것이며 이는 우리 모두가 알듯이 문명의 종말을 가져올 것입니다.
전쟁보다 외교적 해법을 지지하며, 우리는 다음과 같이 요구합니다.
- 우리는 그 어느 측의 선제적 무력 사용에도 반대합니다. 이는 역효과를 낳을 것이며 상황을 핵전쟁으로 이끌 것입니다.
- 우리는 모든 측에 군국주의적 수사와 도발적인 군사 연습을 그만두기를 촉구합니다.
- 우리는 중국, 일본, 북한, 러시아, 한국, 미국이 3+3 방식의 동북아 비핵지대 수립을 위한 단계적이고 포괄적인 접근법을 검토하기를 권합니다. 이는 일본과 한국에서 두 개 이상의 정당의 지지를 받고 있으며 북한 정부도 관심을 가지고 있습니다.
- 우리는 중국, 일본, 북한, 러시아, 한국 그리고 미국이 또한 한국전쟁(1950-1953)을 공식적으로 끝내도록 1953년 정전협정을 전환하기 위한 가능한 옵션과 양식을 고려할 것을 권합니다.
- 우리는 6자회담을 재개하라는 유엔 사무총장의 요구와, 협상을 지원하겠다는 그의 제안을 환영합니다.
- 우리는 이란 핵 프로그램 협상을 성공적으로 이끌었던 것처럼 외교적 협상을 지원하겠다는 유럽연합(EU)의 제안 역시 환영합니다.
- 우리는 유엔 안전보장이사회에 갈등에 대한 외교적 해결을 우선시할 것을 요구합니다.

Appeal for a diplomatic solution in North East Asia
The Abolition 2000 members and affiliated networks listed below, representing peace and disarmament organisations from around the world, call on the United States and North Korea to step back from the brink of war in North East Asia, and instead adopt a diplomatic approach to prevent war.
We call for the immediate commencement of negotiations to prevent a military conflict from erupting, and to resolve the underlying conflicts. Such negotiations should take place both bilaterally and through a renewed Six-Party framework involving China, Japan, North Korea, Russia, South Korea and the United States.
The escalating tensions and threat of military conflict over North Korea's nuclear and missile capabilities makes a diplomatic solution of vital importance and the highest priority. The increasing risk of war - and possibly even the use of nuclear weapons by miscalculation, accident, or intent - is frightening.
More than three million citizens of Korea, China, USA and other countries lost their lives in the Korean War from 1950-1953. Should a war erupt again, the loss of lives could be considerably worse, especially if nuclear weapons are used. Indeed, a nuclear conflict erupting in Korea could engulf the entire world in a nuclear catastrophe that would end civilization as we know it.
In supporting diplomacy rather than war, we:
- Oppose any pre-emptive use of force by any of the parties, which would be counter-productive and likely lead to nuclear war;
- Call on all parties to refrain from militaristic rhetoric and provocative military exercises;
- Encourage China, Japan, North Korea, Russia, South Korea and the United States to consider the phased and comprehensive approach for a North-East Asian Nuclear-Weapon-Free Zone with a 3+3 arrangement*, which already has cross-party support in Japan and South Korea and interest from the North Korean government;
- Encourage China, Japan, North Korea, Russia, South Korea and the United States to also consider options and modalities for turning the 1953 Armistice Agreement into a formal end to the 1950-1953 Korean War;
- Welcome the call of the UN Secretary-General for a resumption of Six-Party talks and his offer to assist in negotiations;
- Welcome also the offer of the European Union to assist in diplomatic negotiations, as they did successfully in the negotiations on Iran’s nuclear program;
- Call on the United Nations Security Council to prioritise a diplomatic solution to the conflict
* The 3+3 arrangement would include Japan, South Korea and North Korea agreeing not to possess or host nuclear weapons, and would require China, Russia and the USA agreeing not to deploy nuclear weapons in Japan, South Korea or North Korea, nor to attack or threaten to attack them with nuclear weapons.

‘아프니까 청춘이다’라는 말이 유행했던 시기가 있었죠. 유행은 지났지만 여전히 취업난에, 경제난에 몸과 마음이 아픈 청년들이 많습니다. 열악한 환경에 있는 청년들이 아프기 전에 미리 사회가 예방할 수 있는 방법은 없을까요?
태어나자마자 가입이 의무라는 국민건강보험공단에서 최근 2017년 각 부문별 건강검진 안내문이 공개되었는데요, 청년 세대의 건강관리는 어떻게 진행하고 있는지 자료를 통해 알아보겠습니다.

먼저 2017년 일반건강검진 안내문에서 대상자 선정 부분입니다. 내용은 다음과 같은데요,

청년세대는 세대주가 아니거나 직장이 없다면 매년 혹은 2년마다 실시하는 기본적인 건강검진에서도 제외가 되고 있었습니다! 장기 불황으로 20세 이상 40세 미만의 인구 중 미취업자 비율이 2017년 2월 현재 33%가 넘어가고 있는데요(통계청, 경제활동인구조사), 10명 중 3명은 세대주가 아니라면 기본적인 건강검진도 받지 못하고 있는 실정입니다.
취업 준비 기간이 장기화되고, 그동안 청년들은 심각한 경제난을 겪게 되어 큰 사회문제가 되고 있는데요, 관련 기사들(하단 링크 참조)에 따르면 경제난을 겪고 있는 청년 중에는 건강보험료를 지속적으로 납부하지 못해 ‘생계형 체납인’이 되는 경우도 있지요. 이들의 경우, 아파도 병원을 찾지 못하는 경우가 많아 질병을 키울 수 있어 우려가 되는 상황입니다.
다음은 국가암검진 인데요, 역시 청년세대(20세-39세)는 자궁경부암 검진을 제외하고는 암 검진을 받을 수 없는 상황입니다. 건강검진 안내문에는 ‘암은 생존율이 높은 초기에 확진을 위해서 주기적인 검진이 필요하다’고까지 친절하게 적혀있지만 20세-39세는 국가암검진 대상자가 아니라니 아이러니합니다.
이에 관련 내용을 건강보험공단의 관련 부서로 문의했는데요, 관련 부서는 청년세대가 암 검진을 받지 못하는 이유로 크게 두 가지 이유를 들었습니다. 첫째는 건강검진제도가 만들어질 때 성인병을 진단하는 데에 주 목적이 있었기 때문에 그 영향이 지금까지 이어지고 있다는 것이었고, 둘째는 건강검진 항목은 국가건강검진원칙에 따라 설정되는데 이 원칙을 따르다보니 청년세대는 암 검진에서 제외된다는 것이었습니다. 한정된 재원으로 국가건강검진 항목을 구성해야 하다 보니 검진으로 인한 이득이 손해 보다 커야 하는데, 청년층은 상대적으로 암 발병률이 장년층보다 낮아 이득이 손해 보다 크다고 말하기 어렵다는 주장이었습니다.
국가건강검진원칙 1.중요한 건강 문제 일것 2.조기에 발견하여 치료가 가능한 질병일 것 2-1.질병을 조기에 진단할 수 있는 정확한 선별검사방법 및 검사주기가 존재 할 것 2-2.조기발견에 따른 근거 있는 치료 및 관리방법이 있고 가능 할 것 3.검진방법이 수용성이 있을 것 3-1.국민이 쉽게 받아들일 수 있는 방법일 것 3-2.인프라가 구축되어 있을 것 (검진기관 수,시설,장비,인력 등) 4.검진으로 인한 이득이 손해보다 클 것 5.비용대비 효과가 있을 것
하지만 이로 인한 문제는 의외로 단순하지 않습니다. 왜냐하면 국민건강보험공단은 암 환자 의료비 지원 사업도 진행하고 있습니다만 의료비를 지원 대상이 되려면 일정 조건을 충족해야 하기 때문입니다. 그중 필수 조건은 ‘국가암검진 사업을 통해 확인된 암환자일 것’입니다. 해당 사업은 건강보험 가입자의 경우 최대 200만 원씩 3년을 지원받는 큰 사업입니다. 2014년도 기준으로 암 발생자수 중 20세-39세가 16,743명(보건복지부, 암등록통계)임에도 불구하고, 이들은 애초에 국가암검진 사업 대상자가 아니기 때문에(자궁경부암 환자 제외) 암에 걸려도 국민건강보험공단에서 암환자 의료비 지원금을 받을 수가 없는 실정입니다. (39세라면 40세에 검진을 받아야 하는 걸까요? ㅠ.ㅠ 만에 하나 암 환자라면 암세포가 계속 자라겠죠..ㅠ.ㅠ)
건강보험공단 담당 부서의 설명대로 재원이 한정적이라 상대적으로 발병률이 낮은 20세-39세의 국가암검진은 검진 주기를 길게 잡거나 미실시한다 치더라도, 암 환자의 의료비 지원 사업에서 나이를 이유로 지원 대상에 차별을 두는 것은 도저히 이해할 수가 없습니다. 오히려 사보험이나 재산이 적은 청년세대에게 이런 제도는 꼭 필요한 것 아닐까요?

건강검진기본법의 기본이념은 모든 국민이 건강위험요인과 질병을 조기에 발견하여 치료를 받음으로써 인간다운 생활을 보장받고, 건강한 삶을 영위하는 것입니다. ‘아프니까 청춘’이 아니라 ‘아프니까 건강검진, 아프니까 의료비 지원!’이 되도록 건강보험공단은 조속히 대책을 내놓아야 합니다.
